Understanding Keratosis Pilaris (KP)
One of the most common conditions I encounter in clinic is keratosis pilaris (KP) or strawberry legs. It affects individuals of all races and sexes, typically presenting in childhood and becoming more prominent during the teenage years - in fact, as many as 50% of us have the genes for KP. However, it shows up to a varying degree in different individuals and often improves as we get older.
Characteristics of KP
KP is characterised by a very typical appearance of redness and bumps on the extensor surfaces of the upper limbs in other words it's largely out of sight. The condition is a result of abnormal epidermal differentiation in the follicle, leading to a plug, which then causes inflammation, scaling, and the formation of a papule. Sometimes, coiled hair is also found. While KP is associated with eczema and other skin conditions, it can occur in isolation in otherwise healthy skin.
How to manage KP
Fortunately, there are ways to manage KP to make it look better. I like to approach the treatment with a combination approach. KP is a barrier issue and a skin cell differentiation issue, so we need to address both aspects to improve the appearance of the skin.
To improve skin barrier health, gentle cleansing and skipping fragrance and physical exfoliants is essential. Regular use of occlusive skin hydrators, such as shea, mineral oil, and silicones, is also helpful as it locks water into the skin’s surface.
To tackle the bumps, we can use several ingredients such as AHAs, including lactic acid, which help exfoliate the skin, and niacinamide, which helps with redness and barrier function. Bakuchiol, a plant-derived ingredient, can also be useful as it helps improve cellular differentiation in a similar way to topical retinoids (although it isn’t actually a retinoid itself). Gentle topical retinoids, which help improve cellular differentiation, are also useful, but it's crucial not to allow skin irritation to develop as it can compound the skin barrier issues, leading to yet more dryness and scaling.
